Occupational Therapy is covered by most insurances when *prescribed by a physician (MD), a psychologist or neuropsychologist (PhD or PsyD), a clinical nurse practitioner (CNP), or a licensed social worker (LISW).

Any other clinical/non-clinical professionals may refer, as needed, eg massotherapist, registered nurse, reiki practitioner, dietician, physical therapist, speech & language pathologist, physical trainer, minister, coach, teacher, etc. Insurance coverage is not guaranteed, but a Super Bill is issued to enable client to apply for reimbursement of fees paid.

(*As such, no prescription is required in Ohio for self-pay, as consumers have “direct access” to Occupational Therapists.)

Scripts should read:

“Occupational Therapy, Evaluate & Treat,” and include the numeric ICD-10 code(s).

If mandated by your policies and procedures, please have client sign your Release of Information form to enable two-way, confidential communication between you and Carol A Leslie, OT/L, ACHT, CWC. You will receive a report outlining her Occupational Therapy Treatment Plan, soon thereafter.

Your patient is best served when a copy of your evaluation is sent to Ms Leslie at the time of referral, or issued to patient to deliver. All relevant paperwork, either narrative summary or assessment(s), may also be mailed to:
